Your First Visit to Eastridge Family Dental

During your first visit, you’ll receive a comprehensive dental exam and X-rays. Dr. Waugh will review your medical and dental history, address any immediate concerns, and work together on a treatment plan for lasting oral health. Our family-friendly office is newly renovated for comfort, making your first visit the start of a long-term relationship. We’re here to answer your questions and provide the dental patient resources you need.

Please bring the following to your first visit:

• Any prior dental X-rays (or have your previous dentist forward them to us). 
• A valid form of ID.
• A list of medications you’re currently taking.
• Information on any health concerns, surgeries, or conditions.
• Your dental insurance card (if applicable).
